Remove unused constants and clean up stale comments

Browse Source 
- Remove TOP_VFO_BTN_H and VFO_BTN_W (unused local consts, compiler warned)
- Consolidate duplicate comment block in AudioOutput.WriteDouble
- Clean up AudioOutput callback comment header
